The PCAM was developed to measure the physical health, mental health, social support, social needs, health literacy and engagement of patients from the point of view of the clinician. There are 12 items across the categories of Health and Well-being, Social Environment, Health Literacy and Communication, and Service Coordination. Although it is copyrighted, the PCAM is free to use and can be found at www.pcamonline.org along with resources and references behind the development of the measure.
