Treatment Timing in a Group of Patients Treated with the Bimler Removable Functional Appliance

Introduction: The objective of the current study was to describe the timing of the treatment with the Bimler Appliance of patients with malocclusion as judged by the CVM method. Materials and Methods: 90 cases, of both genders, were randomly selected from a pool of records donated to the University at Buffalo School of Dental Medicine from the practice of the late HP Bimler in Wiesbaden, Germany. The cases were divided into three groups according to the type of malocclusion; Class II Div1, Class II Div2, and Class III. Cases were randomly selected for each group until a sufficient number (30 per group) of records that met the inclusion criteria were collected. The records were selected to represent both genders (50 Females, and 40 Males). Age at the beginning of treatment, gender, treatment duration, date at which treatment was started, and the initial Lateral cephalogram were collected. A single operator determined the CVM stage for each subject. Results: There was no statistically significant difference (p>0.05) between males and females in age at the start of treatment or duration of treatment. Furthermore, there was no statistically significance difference (p>0.05) between mean age at the start of treatment between the three groups (9y 7m, 10y, and 8y 11m for Class II Division 1, Class II Division II, and Class III respectively), however, treatment duration was significantly longer for the class III group (p<0.05). About 70% of cases in all three groups were at CVM 1 at the start of treatment. Average CVM at the beginning of treatment were 1.47, 1.7, and 1,37 for Class II Div 1, Class II Div II, and Class III respectively; these differences were not statistically significant (p>0.05). Conclusion: There was no evidence that Dr. Bimler used the CVM method to determine the optimal time to start treatment with the Bimler appliance. 70% The CVM stages observed in this investigation for each group was CVM 1 (the average was between CVM 1 and CVM 2).
